Discharge
Discharge frequently occurs between 8:00 am and 10:00 am, but can take place at any time as determined by your doctor. Advance notice is usually, but not always provided the day before, so it is important to have someone "on-call" at all times as you know the day is nearing. Deborah is not able to accommodate patients that have no transportation. We ask your assistance in planning ahead so that another patient's admission is not delayed or cancelled.

Let Us Help You
It is the responsibility of the Case Management, Social Services Department to assist patients with discharge planning. If you are able to care for yourself or have a caregiver that can provide assistance, you probably will not need their help.
However, there are times when your social situation and/or medical condition suggests that you will need the help of the Case Management, Social Services Department in

planning for your discharge. You should receive the help Social Services if you are currently using:

Nurse/Homemaker/Meals on Wheels services which will need to be restarted upon discharge.

You require medical equipment at home (i.e, oxygen). You require medical care in the home that must be provided by a skilled nurse.(i.e, wound care, insulin teaching)

You may need to recuperate in a convalescent facility prior to returning home.

You may need referral services and/or information about community resources regarding your medical and/or financial situation.

If you are currently in the hospital and need the help of the Case Management, Social Services Department please ask your nurse or physician to contact us as early into your hospital stay as possible.
