About Your Stay: When You Arrive
When you get to Deborah, you must register at central registration, located off the parking deck in the rear of the hospital. There will be a schedule with your name on it. Check in at the desk and have a seat until you are called.

While you are waiting, ensure that all necessary documents are handy and forms are signed. When you are called, you will be asked to provide information that will be helpful to ensure payment of your bill by your health insurance. You will sign forms for your insurance, and also sign a form called "Consent for Medical Treatment." You will be given an identification bracelet which will be worn throughout your stay. If you have not had preadmission testing you will likely have an EKG, blood tests and a chest X-Ray. A representative will direct you to the appropriate places for these tests. After the testing, you will be sent to the nursing unit.

Admission Checklist
Please comply with the following:
Identification (drivers license, social security card) and Insurance Cards

Advance Directives (living will, Healthcare Representative, etc.

Arrange in advance for transportation at the time of discharge. This can occur at any time during the day so this individual should be available all day. Discharges usually occur by 10:00 am.

Comply with all insurance requirements (i.e, notify insurance carrier of admission, obtain second surgical opinion)

HMO Referral from your primary care physician (if applicable)
